Abstract

Background : Ultraound Elastography has shown an evolving role in clinical practice over the past 10 years in a tandem with the trend to change the clinical management of chronic allograft fibrosis and dysfunction.Studies has shown that ultrasound sono-elastography has appeared to be useful in identifying and diagnosing chronic allograft fibrosis. Ultrasound sono-elastography has many advantages such as being a fast , cost effective, recently validated technology in the assessment of chronic allograft nephropathy as it is a static method combining direct visualization of renal parenchyma and stiffness measurements .Thus , it is important for physicians to take benefit of each modality for screening and examination of the allograft fibrosis also further studies and trials of the usage of Elastography is needed to get a better validation of their use is diagnosing renal allograft fibrosis. Although ultrasound sono-elastography is a promising tool for the evaluation of renal allograft fibrosis yet renal allograft biopsy is still the gold standard for diagnosing chronic allograft fibrosis and dysfunction. Nevertheless, sono-elastography may be valuable in the follow up and continous observation of the renal allograft fibrosis .transplantation. Nevertheless, the role of UE might be found to be more valuable in the continuous observation and follow-up of allograft fibrosis during check up by physicians.
Conclusion:Strain sono-elastography is a promising imaging modality for the screening and detection of chronic allograft fibrosis ,yet cannot replace the gold standard renal biopsy but it can be used for screening or when renal biopsy is contraindicated due to any reason. And may help physicians making decisions regarding whether to take renal allograft biopsy or not .
