Abstract

Objective: To study the Doppler indices of the main fetal pulmonary artery [pulastility index (PI), resistance index (RI), peak systolic volume (PSV) and acceleration time/ ejection time ratio (At/Et)] and their value in prediction of fetal outcome regarding development of RDS in controlled diabetic cases at age of 37weeks undergoing elective cesarean section.
Methods: A cross section analytic study included 174 pregnant women with controlled Diabetes Mellitus at 37 weeks and will have elective cesarean section, underwent fetal pulmonary artery flow velocity measurements, including pulastility index, resistance index , peak systolic volume and acceleration time/ ejection time ratio were obtained using spectral Doppler ultrasound. Then observing the outcome according to development of neonatal RDS.
Results: There was an inverse relation between At/Et ratio in the fetal pulmonary artery velocimetry waveform and development of neonatal RDS. The cut-off value for At/Et ratio in our study was 0.29 with sensitivity 95.2% and specificity 93%. There was an inverse relation between fetal pulmonary artery RI , PI and fetal lung maturity.
Conclusion: A decreased At/Et ratio in fetal pulmonary artery is associated with subsequent development of RDS in neonates of diabetic mothers.
