Abstract

Sudden unexpected death in epilepsy (SUDEP) is a catastrophic event affects thousands of epileptic patients as it is the most common cause of mortality among epileptic patients. It is ranked second to stroke regarding total years of potential life lost among neurological diseases. The exact pathophysiological mechanism isn’t fully understood, however it is it is usually attributed to cardio-respiratory neural mechanism. Cardiac stiffness is proposed to be one of the possible predisposing factors of SUDEP which causes impairment of diastolic function.
Transthoracic echocardiography is the first non-invasive method for evaluation of diastolic dysfunction and can be used as an indirect indicator of cardiac stiffness. So, the aim of this study is to evaluate diastolic function among patients with temporal lobe epilepsy and compare them to non-temporal lobe epileptic patients and healthy individuals by using transthoracic echocardiography.
We enrolled 66 subjects (18 temporal lobe epileptic patients, 26 non-temporal lobe epileptic patients and 22 healthy individuals) and found statistically significant difference regarding thinner PWT, lower DT, higher E/A ratio and TRV and no statistically significant difference is demonstrated regarding E/E` ratio which is the most sensitive indicator of diastolic dysfunction between the patients group including the temporal lobe and the non-temporal lobe epileptic patients compared to healthy controls.
Post hoc analysis revealed the temporal group had lower DT, higher E/A ratio and TRV compared to healthy controls and the comparison between the non-temporal group and healthy controls showed thinner PWT lower DT, higher E/A and TRV.
