Abstract

Breast cancers in young women tend to be larger, more advanced, more aggressive features and worse outcomes compared to older women. Magnetic resonance imaging (MRI) is by far the most sensitive imaging modality in young high-risk women. We aimed to highlight role of MRI in the diagnostic work up and management of breast cancer in young patients. Results: nineteen patients presented with palpable mass lesion and six patients with mastitis. Nineteen patients had multicentric breast cancer and six patients had single masses. Five patients had positive family history. Twenty one patients had invasive duct carcinoma. Eleven patients were ACR c breast density and 10 patients were ACR d breast density. All twenty five patients received NAC, sixteen patients showed response to chemotherapy while nine patients showed no response to chemotherapy. Post NAC and pre- operative MRI changed management of twenty cases had mastectomy and five cases had conservative surgery. Conclusion: We concluded that DCE-MRI has a vital role in the diagnostic work up and management of breast cancer in young women. It seems probably no longer ethical to offer film mammograms to young women if MRI is feasible
