Abstract

Background: Acral vitiligo is a subtype of vitiligo that is characterized by hypopigmented or depigmented patches of skin that affects acral parts of the body (hands, feet and skin over joints) and it is a resistant subtype that does not respond easily to any treatment modality. Aim of the work: The aim of this study is to evaluate the effect of medium dose UVA1 phototherapy (40-70 J/cm2) in the treatment of acral vitiligo, and comparing it with topical PUVA in the treatment of this resistant type of vitiligo. Patients and methods: Twenty patients with acral vitiligo (10 males and 10 females) were recruited from the dermatology outpatient clinic in Kasr Elainy, Cairo university, patients divided into two groups with each group included 10 patients, group (A) received medium dose UVA1 phototherapy (40-70 J/cm2) and group (B) received topical PUVA three times weekly for 12 weeks. Every patient was clinically evaluated monthly as regards: extent of body affection and appearance of new lesions or increase in diameter of the current lesion according to VASI and VIDA scores. Results: Within group (A), it was found that one patient (10%) had a poor response in the form of diffuse tanning while 9 patients (90%) showed no response. Within group (B), it was found that 10 patients (100%) had no response. Comparing both groups regarding response and pattern of response, no statistically significant difference was found as regards response (p-value>0.05), and as regards pattern of response P-value>0.05
