Introduction:Fat injection is a rapidly evolving concept in breast surgery. Different techniques have been modified in an attempt to achieve the best results. Different modalities of harvesting, processing and injection of the fat graft have been reported by different surgeons, with different graft retention percentages. Objective:The effectiveness of fat graft retention has been the subject of different researches, using various assessment modalities. Patients and methods: This study has been done on 40 cases. 65% had aesthetic indication, 35% were of reconstructive indication. A fixed technique of breast fat injection was used in all cases. Harvesting using multi-hole blunt 3mm cannula, with suction of - 0.5 atmospheric pressure was used. Fat was processed by sedimentation for of 15-20 minutes. Injection done using a 2mm single hole blunt cannula connected to 5 cc syringes. Follow up and assessment of fat graft retention volume was done using preoperative and postoperative imaging .Mammograms accounted for 72.5% and MRI for 27.5%. Results:Average retention volume was calculated to be 293.9 cc which is 54.5% of the injected fat. 57% was calculated in the aesthetic group and 48% was calculated in the reconstructive group. Conclusion: the use of imaging modalities as an objective method of assessment of breast fat injection proved to be highly accurate at assessment of volume retention and is highly informative of the quality of fat retained
