Abstract

x Background: To evaluate value of vaginal fluid creatinine in the diagnosis of cases of preterm premature rupture of membranes among patients with history of amniotic fluid leak. x Methods:A total of 100 pregnant women with Gestational age 24 : 37 weeks gestation participated in this study divided into 2 groups. group I (confirmed PROM group) & group II (normal non PROM group). All patients underwent a sterile speculum examination for detection of amniotic fluid pooling in the posterior fornix followed by vaginal fluid sampling (3 cm). Creatinine detection was done using ELISA. x Results: The mean vaginal fluid creatinine levels in group (I) & group (II) were .70 ± .88 and .04 ± .18mIU/mL respectively, The difference was statistically significant (P value <0.001). With creatinine cut-off value of 0.25 mIU/ml, the sensitivity& specificity in confirming PROM are 72, 94% respectively. x Conclusion: Creatinine level was significantly higher in cases with definite PROM. Consequencely, vaginal fluid creatinine can be used as easy, rapid, reliable and non-invasive test for confirming the diagnosis of PROM and can be used as a adjunctive test in equivocal cases
