Abstract

Background: Soluble transferrin receptor (sTfR) is one of the main regulators of cellular iron homeostasis, and is an emerging diagnostic tool of iron status. Objectives: to investigate the diagnostic efficiency of sTfR as compared to serum ferritin in the assessment of iron overload expressed as liver iron concentration (LIC) measured by magnetic resonance imaging (MRI) in patients with chronic hemolytic anemia (CHA). Patients and methods: Eighty CHA patients were recruited; their mean age 10.8±4.5 years. Serum ferritin and sTfR levels were assessed for all patients. LIC was extracted from patients{u2019} files. Results: Mean sTfR was121 nmol/L and it was comparable between TM and SCD group (p>0.05), mean serum ferritin was 2478 ng/ml; and mean LIC was 18.6 mg/g dw. LIC was significantly higher in TM group (p<0.001). STfR didn{u2019}t correlate with serum ferritin or LIC (p=0.273, 0.725 respectively), but positive correlation was found between serum ferritin and LIC (r=0.49, p=0.001). Patients with LIC >7mg/g dw had higher median sTfR and serum ferritin compared to group B (p=0.026 and 0.003 respectively). Both sTfR and SF were significantly good predictors of iron overload (p{u02C3}0.05). Cohen’s kappa testing showed agreement between Serum sTfR and LIC and between serum ferritin and LIC in the assessment of iron overload; but this significance was lost at high serum sTfR values {u2265}133.3 nmol/L. Conclusion: soluble TfR was a valuable quantitative assay of iron overload in children with chronic hemolytic anemia. However, its overall accuracy as well as agreement with LIC was lost at high serum values
