Abstract

Von Willebrand factor (VWF) is a multimeric glycoprotein which is essential for primary hemostasis and plays central role in thrombosis. It contributes to the risk of transient ischemic attack (TIA)/stroke in patients with cerebrovascular disease. Optic nerve ultrasonography has been proposed as a noninvasive, quick method for diagnosing increased intracranial pressure by measuring optic nerve sheath diameter.
Our study is case control study that was conducted in Kasr Al Ainy Stroke Unit, (Cairo University Hospitals. Our study included 90 subjects; 30 acute ischemic atherosclerotic stroke patients, 30 acute cardioembolic stroke patients and 30 age and sex matched normal healthy control subjects. All stroke patients were recruited within 1 week from onset of stroke. . All subjects underwent complete neurological examination, extra and transcranial duplex ultrasonography (U/S), optic nerve ultrasonography, CT brain, routine laboratory work up and serum level of vWF.
Positive association between VWF plasma level and acute ischemic stroke was observed in this study. High plasma level of vWF was observed in patients with acute onset of ischemic stroke during first week from stroke onset. High vWF plasma level was observed in patients with acute atherosclerotic stroke than in acute cardioembolic stroke patients.
We observed significant increase of ONSD in patients with acute ischemic stroke. More increase of ONSD on the ipsilateral side of infarction was found.
Significant association between ONSD and degree of stenosis of the intracranial arteries was detected. High degree of stenosis was associated with more increase of ONSD. However, there was no association detected between ONSD and degree of stenosis of extracranial arteries.
We found significant association between ONSD and DM, patients who were diabetic showed more increase in ONSD compared to non-diabetic patients. Significant relation between ONSD and NIHSS score was detected, patients with higher NIHSS score had more increase in ONSD. Positive relation between ONSD and MRS score was found, patients with high MRS score had greater ONSD.
