Conclusion
A significant improvement was observed among mother’s knowledge and practices post discharge plan program. There was a strong statistically significant correlation between the total mother’s knowledge and total reported practice score on the post-test (P<0.01**). There was a strong statistical significant relation between the mother’s knowledge and practices during post implementation of discharge plan (P<0.01**). The most of those mothers with good total knowledge scores had satisfactory levels of practice. Finally, the present study finding had stressed that discharge plan program has positive influence on improvement mother’s knowledge and practice for their children undergoing congenital heart surgery and then research hypothesis was achieved.
Recommendations
Based on the current study findings the following recommendations were proposed
• Implementation of Comprehensive, multidisciplinary discharge planning to mothers and their children contain education regarding congenital heart surgery care.
• Providing mothers with discharge plan of their children after congenital heart surgery should become a routine practice to congenital heart surgery department to enhance fast recovery and prevent complications.
• Discharge care plans to promote optimum development of the child and focus on normalization, the impact of the child’s health status, and requirement of mothers according to educational needs.
• Encourage mothers to share their opinions and share medical staff to take decisions about child care.
• Enhancing mother’s practices level toward care of children with congenital heart surgery about breast feeding, bottle feeding immunization, oral care, and incision care.
• More researches about improving mothers discharge and accepting responsibility for caring of their children with congenital heart surgery.
• Further researches to examine the importance of discharge planning for congenital heart surgery for mothers and children.
Summary
A
congenital heart defect (CHDs) is a defect structural with the heart present at birth due to heart or blood vessels don’t develop normally before birth, its range in severity from simple problems to very sever malformations, such as complete absence of one or more chambers or valves. Congenital heart defects are the most common birth defects. CHDs are a leading cause of birth defect-associated infant illness and death. The second cause of death in infancy and childhood, and are the leading causes of childhood morbidity and mortality especially in developing countries. About 25% of babies with a CHD have a critical CHD. Infants with critical CHDs generally need surgery or other procedures in their first year of life.
Congenital heart defect surgery corrects a heart defect that a child is born with it. Surgery is performed if the defect could harm the child’s long-term health or well-being. Heart surgery in children is carried out to correct and repair con-genital heart defects and heart diseases needed surgery and hospitalization. The surgery is very important for the child’s well-being.
Discharge care planning is the development of a personalized plan for the child who is leaving a hospital, that aim to enhance mothers’ discharge readiness and improve children outcomes. Discharge care is the role of a nurse. The nurse is responsible for providing mothers with adequate knowledge and instruction about the care of their children.
Discharge planning in pediatric nursing can improve mothers’ knowledge and confidence before discharge, reduce the readmission rate of the children, and promote the mother’s caring competency.
The present study aimed to evaluate the effect of discharge plan for mothers and their children undergoing congenital heart surgery through, assessing mothers’ knowledge regarding care for their children undergoing congenital heart surgery. Assessing mother’s reported practices regarding care for their children undergoing congenital heart surgery. Designing and implementing discharge plan for mothers and their children undergoing congenital heart surgery. Evaluating the effect of discharge plan for mothers and their children undergoing congenital heart surgery. Discharge plan will improve the knowledge and practices of mothers and their children health status undergoing congenital heart surgery.
This quasi experimental research design was conducted at conducted in outpatient clinics of Cardio Vascular and Thoracic Academy Ain Shams University Hospitals, 70 mothers were included in this study. Two tools will be used for data collection to achieve the aim of the present study: First tool an interviewing questionnaire for mothers include the following. First part: Socio demographic characteristics of the mothers such as (age of mothers, educational level, and occupation, marital status, Children demographic characteristics as age, sex, Birth order, child past and present medical history and type of Surgery…etc. Second part: Assessment of mothers’ knowledge regarding care of their children undergoing congenital heart surgery. Such as (activities, rest pattern, wound care, nutrition, medications, warning signs …etc.
Third part: Mothers’ reported practices regarding caring of their children undergoing congenital heart surgery as (Incision care and prevention of infection, Child nutrition, oral care, medication administration, immunization, the follow up system…etc.
Second tool: Assessment of child health status assessment of child health status through physical examination of child growth and development using Denver developmental screening test (Berman, 2012).
The result of this study concluded that:

The study recommended that:
• Providing mothers with discharge care of their children after congenital heart surgery should become a routine practice to congenital heart surgery department to enhance fast recovery and prevent complications.
• Discharge care plans to promote optimum development of the child and focus on normalization, the impact of the child’s health status, and requirement of mothers according to educational needs.
• Encourage education programs on importance of pre-marital genetic counseling facilities screening program to control of genetic diseases.
• Encourage mothers to share their opinions and share medical staff to take decisions about child care.
