Abstract

Nasal obstruction often results from inferior turbinate hypertrophy. It commonly results from allergic rhinitis or chronic vasomotor rhinitis. Additionally, it might happen as a coping mechanism on the other side of a deviated nasal septum. About 10–20% of allergy patients report chronic nasal blockage brought on by inferior turbinate hypertrophy.. Surgical interventions for inferior turbinate hypertrophy target excision of the hypertrophied tissue. The tissue to be resected may involve the soft tissue, bone or both. The goal is to provide resolution of nasal obstruction as well as objective and subjective improvement. This systematic review conducted on 17 studies enrolled patients with turbinate hypertrophy refractory to medical treatment comparing the clinical outcomes of the different surgical modalities of inferior turbinate reduction. The outcome measures considered: Postoperative reduction of nasal obstruction, post operative nasal symptoms and healing. Regarding comparison between radiofrequency and microdebrider, there was significant reduction in nasal obstuction in patient underwent microdebrider compared to those underwent radiofrequency; however, the effect estimate was statistically nonsignificant. While regarding saccharin test (one of the various methods that have been used to assess mucosal damage in the literature), both techniques had similar results. Concerning comparison between radiofrequency and surgery, We found significant reduction in degree of nasal obstruction in patient treated with radiofrequency compared to those treated with submucosal resection. On the other hand, patient treated with ultrasound turbinate reduction had better results regarding nasal obstruction than those treated with radiofrequency. With reference to comparison between radiofrequency and submucosal cauterization, Nasal obstruction degree was similar in both techniques. With reference to comparison between radiofrequency and electrocautry, three studies compared radiofrequency to electrocautery using visual analogue scale, the pooled estimate showed there is no significant difference between both techniques, but in kilavuz2014 electrocautery had better outcome. Also, Saccharin test was similar in both techniques. According to the incidence of crusting, there was no significant difference between these techniques when compared to each other’s; Radiofrequency and surgical submucosal resection, outfracture and submucosal cauterization, diathermy + outfracture and submucosal resection, inferior turbinectomy and submucosal diathermy. The only technique that had low crustation incidence was microdebrider when compared to partial turbinectomy. According to the incidence of Sneezing, it was found that radiofrequency had significant reduction in the incidence of sneezing compared to submucosal resection. On the other hand, submucosal resection had reduced incidence of sneezing compared to submucosal diathermy. But, there was no significant difference between radiofrequency and microdebrider. Concerning hyposmia, there was no significant difference between radiofrequency and submucosal resection, radiofrequency and microdebrider in the incidence of hyposmia. As regard headache, there was no significant difference between radiofrequency and submucosal resection, radiofrequency and microdebrider in the incidence of headache. According healing, outfracture was compared to submucosal cauterization and submucous resection regarding healing, there was no significant impact between these techniques on the healing process. Concerning Nasal Obstruction Symptom Evaluation (NOSE) score, Radiofrequency alone had lower NOSE score compared to radiofrequency and outfracture. Similarly, submucosal resection lower NOSE score compared to diathermy and outfracture.
