Abstract

Bipolar disorder (BD) is a disabling and chronic
neuropsychiatric disorder, which is characterized by a complex
illness presentation and episode recurrence; it includes a series of
signs and symptoms, the defining characteristic being
pathological mood changes. BD is highly comorbid with
psychiatric and medical disorders. Among the first, the association
between BD and OCD has been widely described.
The current study aimed to assess the occurrence of OCS in
patients with BD, to study the clinical profile of OCS in BD and
to compare OCS profile in manic episodes and in depressive
episodes of BD.
One-hundred BD patients were recruited according to
DSM-4, they were classified into two groups: bipolar depression
and bipolar mania. Full personal history recording was performed
then, SCID-I was conducted to diagnose BD and the exclusion of
other psychiatric disorders. Y-BOCS was used to diagnose OCD
and assess severity of it and finally, SCID-II was used to diagnose
an axis II diagnosis.
Of the included patients, only 30% had OC symptoms
comorbid with BD. Bipolar-OCD patients (30 patients) were 22
males and 8 females with mean age 35.5±10.5 years. Twelve
patients were single, 12 married and 6 divorced. On the other

hand, bipolar without OCD patients were 53 males and 17
females with mean age 38.4±10.1 years. Twenty-nine patients
were single, 30 were married ,10 divorced and 1 widow. Family
history of psychiatric disorders statistically was significantly
more frequent in cases presented with bipolar-OCD patients.
Regarding clinical data of BD among the studied cases, mean
age of onset of disease was 24.3±8.0 while mean duration of disease
was 13.1±8.8 years. Eighty percent of patients were on antipsychotic drugs plus mood stabilizers. Past history of hospitalization
statistically was significantly more frequent in cases with mania.
There were no statistically significant differences between mania
and depression regarding demographic data.
Y-BOCS assessment revealed that Patients with comorbid
OCD were more like to occur during depressive episode
representing 73.3% of cases, symmetry was the most frequent
obsession type representing 63.3% of cases while checking was the
most frequent compulsive type representing 73.3% of cases.
Majority of cases (60%) had mild symptoms, in addition mean
obsession score was 7.8±2.9, the mean compulsive score was
7.8±3.4, and the mean Y-BOCS scale was 15.6±5.9. No statistically
significant differences between manic and depressive episodes of
bipolar disorder regarding obsession score, compulsive score, and
total Yale-Brown Obsessive-compulsive score.
