Abstract

Background:
Ankle sprain is one of the most common injuries encountered in
work and sport; and when untreated or badly managed, it can result in
chronic lateral ankle instability.
Objectives:
The aim of this work was to evaluate the diagnostic role of the
ankle arthroscopy in management of chronic lateral ankle instability
and to evaluate clinical and functional outcomes of
repair/reconstruction of the lateral ligament of the ankle for the
management of chronic lateral ankle instability.
Methods:
This prospective study included 24 patients (with chronic lateral
ankle instability) evaluated by diagnostic ankle arthroscopy prior to
repair/reconstruction of lateral ligament of the ankle between June
2020 and June 2022. All patients were postoperatively evaluated
according to VAS score, range of motion and functional (AOFAS)
scoring system.
Results:
A: Diagnostic arthroscopy revealed lesion of the anterior talofibular
ligament in 24 (100%) patients, torn anterior inferior tibiofibular
ligament in 16 (66.6%) patients, synovitis in 17 (70.8%)
patients, osteochondral defect of talus in 8 (38.3%) patients, loos body
in 2 (8.3%) patients and anterior tibial osteophytes in 1 (4.2%) patient.
B: statistical analysis showed a significant 6-month postoperative
improvement in range of motion; and VAS and AOFAS scores.
Conclusion:
On management of patients with chronic lateral ankle instability,
diagnostic ankle arthroscopy should be routinely performed to exclude
the commonly concurrent intra-articular lesions which, when
overlooked, might compromise the postoperative outcomes. In
addition ,open repair/reconstruction of lateral ligament of the ankle
can yield satisfactory postoperative outcomes in forms of pain, range
of motion and functional scoring.
