Abstract

The changes in body structure induced by obesity result not only in increased anatomic difficulties of access for both the surgeon and the anesthesiologist but also in important physiological and pharmacological modifications, which can potentially affect the pharmacokinetic and pharmacodynamics profiles of the anesthetic drugs, these conditions make the obese patient a true challenge to the anesthesiologist..The definition of obesity is based on the degree of excess actual body weight from the ideal body weight ( IBW) for a certain height. Two methods are widely used to define obesity, one is Body Mass Index (BMI=weight kg /height m2), BMI less than 18-24.9kg/m2 is normal, 25-29.9kg/m2 is overweight, 30-34.9kg/m2 is obese class I, 35-39.9 kg/m2 is obese class II, morbid obesity or obese class III > 40kg/m2..Another method is to determine the percentage increase of total body weight (TBW) from IBW; an individual is considered obese when the actual body weight exceeds 120% of the IBW. Determination of the IBW is based on the anthropometric parameters to the following formulae
