Abstract

Background : chronic inflammation of the lung is a characteristic finding in chronic obstructive pulmonary disease (COPD). Leptin is a pleiotropic cytokine thought to play a role in host response to inflammation. As recent studies have shown that leptin receptors are present in the lung, this study aimed to determine if leptin is detectable in induced sputum of COPD stable and exacerbation patients if there is a relationship between sputum leptin and other inflammatory markers in serum Aim of the work: This study aimed to detect sputum leptin and serum TNF-alpha and CRP levels in COPD patients without weight loss during stable disease and acute exacerbation, and to investigate relationships between sputum leptin, serum TNF-alpha and serum CRP in COPD exacerbation.Subjects and methods: 20 stable COPD patients, 20 COPD patients with acute exacerbation and 12 control subjects participated in this study. To eliminate the effects of sex differences, all patients and controls were male. BMI, percent ideal body weight (19-29), percent , TLC, ESR and sputum leptin, serum TNF-alpha and serum CRP levels in day 0 and day 7 were measured in exacerbation and only one in stable patients and healthy individuals (control). Sputum and serum Leptin, TNF-alpha and CRP levels were measured by ELISA.Results: In chronic obstructive disease exacerbation was statistically significant difference between sputum, serum leptin TNF-Ü and CRP levels, before and after treatment. Sputum leptin and serum CRP levels were significantly higher in the patients exacerbation than in the stable patients and healthy individuals (controls). Additionally, Sputum leptin, TNF serum and CRP levels were significantly higher in the patients exacerbation than in the healthy individuals (controls). As regards the correlation between sputum, serum leptin, TNF-Ü and CRP levels with COPD stable patients and healthy individuals (control), it was found insignificant
