Abstract

Plantar fasciitis is a common disorder of the foot and ankle, characterised by pain and stiffness in the heel. The underlying aetiology remains poorly understood, but it is believed that it is due to degenerative changes predisposed by recurrent trivial trauma that can lead to micro tears and inflammatory process. Most of cases are self-limiting that respond to traditional treatment such as rest, analgesics, stretch exercise and shockwaves. However, corticosteroid and PRP provide good effect in non-responsive cases. Surgical role is limited to the remaining few resistant conditions.
Platelet-rich plasma (PRP) is an orthobiologic that has recently gained popularity as an adjuvant treatment for musculoskeletal injuries. It is a volume of fractionated plasma from the patient’s own blood that contains platelet concentrate. The platelets contain alpha granules that are rich in several growth factors, such as platelet-derived growth factor, transforming growth factor-β, insulin-like growth factor, vascular endothelial growth factor and epidermal growth factor, which play key roles in tissue repair mechanisms. PRP has found application in surgical fields to enhance bone and soft-tissue healing by placing supra-physiological concentrations of autologous platelets at the site of tissue damage. The relative ease of preparation, applicability in the clinical setting, favourable safety profile and possible beneficial outcome make PRP a promising therapeutic approach for future regenerative treatments.
This is a prospective interventional study included 30 patients from orthopedic outpatient clinic in Benha university hospital in the period from September 2020 to February 2021. Half of them injected with corticosteroid and the other with PRP.
Data base and confirmed consent of all patients documented. Both groups injected under aseptic circumstances. No complications observed.
Follow up visits arranged at 3 weeks and 3 months using the RM scale, VAS and AOFAS. Statistical analysis designed to compare the effect of corticosteroid and PRP treatment, and correlate the results with possible influent factors.
In our study, all patients improved after treatment with corticosteroid group showed maximum response at 3 weeks, however steroid effect started to decline after that. On the other hand PRP group continued to improve to exhibit better results at 3 months than steroid group.
Previous studies were conducted to detect and compare results of treating plantar fasciitis by corticosteroid and PRP. Consensus is that both have powerful effect with preference of PRP for long term treatment of the disease.
We concluded that local injection of corticosteroid and PRP give good results in relieving pain, PRP showed extended effect renders it superior choice for long term control.
