Abstract Cardiovascular magnetic resonance (CMR) is a powerful diagnostic tool for the evaluation of cardiac diseases through the use of Gd-based contrast agents. In the setting of myocardial scar due to infarction, the inversion-recovery cardiac MR technique allows visual identification of the extent of myocardial scar compared with normal myocardium. In case of diffuse fibrosis, the inversionrecovery cardiac MR technique is unlikely to reveal the presence of diffusely abnormal tissue given the lack of normal myocardium as a reference.
