Abstract

Background: Multiple sclerosis (MS) is a chronic disease in the central nervous system which causes a number of physical symptoms including impairments of motor functions. Purpose: to investigate the effect oflow frequencyrepetitive transcranial magnetic stimulation (LF-rTMS) on motor functions of the lower limbs and to assess the correlation between muscle strength variables and functional performance in MS patients.It aims also to assess the long-term effect of (LF-rTMS)on motor functions in MS patients. Methods: Thirty medically stable remitting relapsing MS patients. Their ages ranged from 20 to 40 years were randomly assigned into two groups (2A3 study and 2B3control). Patients in the study group were treated by (LF-rTMS) on motor cortex (hot spot) ipsilateral to the weaker lower limb in addition to a selected program of therapeutic exercises while patients in control group were treated only by the same program of therapeutic exercises. Treatment was conducted five times per week for two consecutive weeks. Isokinetic dynamometer measurements including peak torque (PT), peak torque/ body weight (PT/BW), time to peak torque (TPT), total work (TW) and average power (AP) of hip and knee flexors and extensors in addition to lower extremity functional performance were assessed and compared for both groups pre-treatment, immediately post-treatment and after four months follow up (long term) using biodex isokinetic dynamometer and Five Repetition Sit to Stand Test (5STS) respectively
