Abstract

Background: Bracing is a critical component of the current standard of treatment for clubfoot. Adherence to the bracing protocol is the main factor for the long term success of the treatment. Parental compliance can be improved by educating the parents as to the proper use of bracing and the hazards of improper or insufficient bracing. Objectives: To detect short-term comparative results of the use of external rotation above knee brace to the dennis brown device in maintenance of corrected CTEV by ponseti casting. Patients and methods: This randomized comparative single blind study involved 60feet with idiopathic CTEV in 38 patients presenting to outpatient clinic in Abu Elrish hospital (pediatric orthopedic surgery unit, Cairo university hospitals) during the period from March 2018 to May 2019. The average follow-up period of each patient was 12 months (±2months). 22 of patients were bilateral CTEV and 16 of them were unilateral CTEV, while 24 of patients were males and 14 were females. In our study, we followed an envelope-randomized method where patients were assigned randomly into two groups: group A: bracing with external rotation above knee brace (30 feet). group B: bracing with Dennis brown device (30 feet). Results: Mean final pirani score in group A was 0.42, while in group B was 0.57(better final clinical assessment in group A)(p value.411). In group A: 13.3% of patients had bad compliance results, while 86.7% had good and intermediate compliance results. In group B: 33.3% of patients had bad compliance results, while 66.7% had good and intermediate compliance results (p value.118)
