Abstract

Chronic anal fissures (CAF) is a troubling and painful condition that affects a great majority of population. Its current clinical management options are either operative or non-operative treatment. Innovative prospects for non-operative medical treatment have been lately employed taking in consideration reduction of the internal anal sphincter (IAS) muscle tone. Phosphodiesterase inhibitors (PDEIs) have proven to cause a dose-dependent reduction in the tone of the internal anal sphincter muscle and promote wound healing. Dipyridamole (DP) as phosphodiesterase inhibitor was selected for topical rectal application in the treatment of anal fissures. As DP is a poorly soluble drug, enhancing its solubility represented a rate limiting step for its formulation.Therefore, microemulsion (ME) technique was used to provide a proper enhancement for DP solubilization. Dipyridamole ME formulations were loaded on to thermosensitive in-situ gel dosage form to facilitate rectal administration. Poloxamer 407 (P 407) was used as the thermosensitive polymer as it exhibits sol-gel transition at body temperature (37{u00B0}C).The prepared in-situ gel formulations were provided with mucoadhesive polymers as Sodium alginate (Na Alg.) and Hydroxypropyl methyl cellulose (HPMC) to prolong the residence time of the prepared formulation at the site of action
