Abstract

Timing of surgical evacuation of spontaneous supratentorial intracerebral hematoma (ICH) is controversial. The aim of the study was to compare the clinical outcome following early versus delayed surgical evacuation of spontaneous supratentorial ICH >30 cc in patients with Glasgow coma scale (GCS) 8-12. The study included 70 patients divided into 2 groups; group A (evacuated within 8 hours) and group B (evacuated later than 8 hours). The outcome was assessed at discharge and 2 months postoperative by extended Glasgow outcome score. Better clinical outcome was found at discharge in the early evacuation group, with no significant difference in the rate of reoperation or the length of hospital stay. We conclude that early surgical evacuation has better clinical outcome in the early postoperative period in patients with good preoperative GCS
