Abstract

Hyperphosphatemia is a major cause of morbidity and mortality in patients with CKD. Aim of work: This study aims to evaluate the effect of controlling hyperphosphatemia on the level of 25(OH) vitamin D and Fibroblast growth factor 23 on chronic kidney disease patients. Patients and methods: 20 CKD patients, stages 3-5 (not on dialysis) defined according to the united States National Kidney Foundation Kidney Disease Outcomes Quality Initiative clinical practice guidelines and who have elevated serum phosphorus were recruited from nephrology outpatient clinic. Results: There was significant increase in the level of 25(OH)vitamin D in two thirds of patients after normalization of serum phosphorus. There was a trend of serum FGF23 to decrease after normalization of serum phosphorus. There was a significant correlation between FGF23 and PTH after phosphate correction signifying the important implication of serum phosphorus normalization. Conclusion: This study casts doubt on the diagnostic value of serum level of 25 (OH)vitamin D in the assessment of vitamin D status of pre-dialysis CKD patients
