Background: Familial Mediterranean fever (FMF) is an autosomal recessive auto-inflammatory disease. Arthritis in early-onset FMF is a common finding. Objective: The aim of the study was to assess the frequency of arthritis, its clinical characteristics and its effect on the disease severity in a cohort of Egyptian children with FMF. Methods: Two hundred patients diagnosed with FMF being followed in Pediatric Rheumatology clinic, Cairo university, were studied. Analysis of joint affection was done regarding its clinical characteristics, response to colchicine treatment and its effect on the disease severity. Results: The frequency of arthritis among our patients was 20.5%. Arthritis was monoarticular in 73.1% and oligoarticular in 22% affecting mainly knees )78%(. The mean duration of arthritis attacks was 110.05 ± 50.9 hours. Arthritis was the presenting symptom in 4% of all patients. Redness of the affected joints was observed in 70.7%. Persistence of swelling after the attacks for 1- 2 days was observed in 17.1%. Limitation of movement during the attacks and complete resolution without chronic limitation was observed in all patients with arthritis (100%)
