Abstract This prospective study aimed at comparing between the effect of intracoronary bolus followed by intravenous infusion of tirofiban & intravenous tirofiban only as bailout therapy on the mortality and morbidity during in-hospital stay. Moreover, left ventricular systolic function was also assessed by 2-D echocardiography at cardiology department, Tanta University. The study was conducted on 150 STEMI patients that were divided into 2 groups. group 1 consisted of 75 patients that had intracoronary bolus followed by intravenous tirofiban infusion as a bailout therapy & group 2 consisted of 75 patients that had intravenous tirofiban infusion as a bailout therapy. Both groups were treated with primary PCI as revascularization technique. It was shown that final TIMI flow and MBG were significantly better in group I than in group II. Moreover, CHF and re-infarction complications occurred significantly more in group II than group I, other MACE (mortality, stroke, and cardiogenic shock), showed no significant difference between both groups. During hospital stay, more group I patients suffered from major bleeding than those of group II, same number of patients suffered from CIN in both groups with no statistical significance regarding these complications. The two groups were assessed as regards lab investigations and echocardiographic parameters at discharge that included measuring EF by Mmode that showed significant difference between both groups. |
