Abstract

Hypospadias is a relatively common male genital deformity affecting about 1 in 300 lives male births and its incidence is increasing. In this condition, the urethral opening is sited anywhere along the underside of the penis from the glans to the perineum.The aim of the hypospadias repair, for which over 200 different techniques are described, is to create a functional neourethra, correct any curvature, and produce a cosmetically normal penile appearance, with a slit-like meatus at the tip of the penis Despite continued refinement of numerous repair techniques, there is no completely satisfactory technique in terms of complications and cosmesis Urethral circumferential dissection and advancement has advantage of this technique is the decreased incidence of development of urethra-cutaneous fistula. However, it carries the risk of development of chordee secondary to taut urethra, potential injury to the urethra during dissection, or development of ischemia from extensive dissection The current study was conducted to evaluate the efficacy of the AUGA technique in the treatment of more proximal degrees of hypospadias with selective criteria and a modified technique. 43 patients, with a range of age between 1-21 years with a mean of 5.67 year were enrolled in the study It was found that 39 (90.7%) subjects had incomplete prepuce. Sites of meatus were glanular, coronal, sub-coronal and distal penile in 11 (25.6%), 9 (20.9%), 14 (32.6%) and 9 (20.9%) patients, respectively. Majority (83.7%) of those patients didn’t develop any complication Single stream was present in 36 (83.7%) patients. Fistula wasn’t reported in the current study while only two patients developed retraction and another 5 (11.6%) patients had wound dehiscence due to infection. Mean postoperative hypospadias objective scoring evaluation score (HOSE) was 14.56 ± 1.29 with a range between 11 and 16. It was found that 36 (83.7%) patients had good appearance while the other 7 (16.3%) patients had poor appearance.
