Abstract Patient and methods: 3 Groups of patients; 1st group compare Early vs. Late cases underwent lobectomy. 2nd group is a comparison study between VATS and open thoracotomy for management of segmentectomy for stage I Non-Small Cell lung cancer (NSCLC) where 91 cases underwent VATS segmentectomy versus 102 cases underwent open thoracotomy, both 2 studies were done at Weill Cornell Medicine, New York in USA while in NCI, Egypt there were 20 cases who underwent VATS biopsy and lobectomies.Results: Patients who underwent VATS had similar sex distribution and similar clinical stages as thoracotomy group. No significant differences were found in the final pathologic stages (p=0.439), total number of lymph nodes sampled (7 vs. 8, p=0.104). VATS was associated with decreased length of stay LOS (4 vs. 5, p=0.001) and lower pulmonary complications (13.2% vs. 26.5%, p=0.022).Conclusion: VATS is a safe and oncologically effective technique for the treatment of early stage lung cancers. VATS patients had a shorter LOS, fewer pulmonary complications, equivalent lymphadenectomy results, and similar oncologic outcomes compared with patients undergoing thoracotomy |
