Abstract

Background: Melasma is a common acquired skin disorder that is characterized by dark, brown symmetrical patches of hyperpigmentation involving the sun exposed areas of the face. Treatments for melasma are generally aimed at inhibiting the pathways that synthesize melanin and decrease of melanosome transfer from melanocyte to keratinocytes, although many treatment modalities are available treatment of melasma remains a challenge. Tranexamic acid (TXA) has emerged as potential treatment for melasma. Different forms of TXA (i.e. oral, topical and intradermal microinjections) have shown promising results. Objective: To evaluate and compare the efficacy of oral versus 4mg/ml and 100mg/ml dilutions of intradermal TXA in melasma. Methods: A randomized controlled trial included a total of forty five female patients with bilateral, symmetrical melasma. The enrolled patients were randomly assigned into three groups. group A (oral TXA), group B (100 mg/ml intradermal TXA) & group C (4 mg./ml intradermal TXA ). group A received oral TXA 250 mg twice daily for 8 weeks, group B and group C received intradermal microinjections every two weeks for 8 weeks. follow up was done 1 month after cessation of treatment
