Abstract

Background: Renal affection in beta-thalassemia major may occur before the occurrence of overt renal symptoms and can be attributed to chronic anemia, iron overload and deferoxamine therapy. Objectives: The study aimed to investigate the signifi»cance of neutrophil gelatinase-associated lipocalin (NGAL) for the early diagnosis of kidney injury in pediatric thalassemic patients having normal blood urea and serum creatinine levels. Methods: Sixty one patients with {uF062}-thalassemia major (mean age 13.75± 4.36 years) compared to 62 healthy age and sex matched controls (mean age 13.55±5.87 years). Patients and controls were subjected to full history taking and clinical examination. Laboratory investigations; included blood urea, serum creatinine, electrolytes, and ferritin were measured and microalb-uminuria, urinary creatinine, calcium, phosphorus, uric acid and urinary NGAL in fresh second morning urine samples also were measured. Results: Urinary NGAL level was significantly elevated in thalassemia patients compared to control group (p<0.001). There were significant positive correlations between urinary NGAL and weight, height, duration of blood transfusion , estimated glomerular filtration rate (eGFR), urinary Ca/Cr ratio, urinary Po4/Cr ratio and urinary albumin to creatinine ratio (ACR) (p<0.05 for all). There was microalbuminuria in 19 (31.1%) of our patients. The ROC curve of NGAL in comparison to microalbuminuria shows the area under the ROC curve was 0.689 , the sensitivity was 89.5% , the specificity was 52.4 and the negative predictive value was very high (91.67%)
