Abstract Introduction: treatment of intraarticular calcaneal fractures remains challenging, with overall high complications rate. ORIF through Extensile lateral approach carries high risk of skin complications. Sinus tarsi approach has gained popularity due to the ability to visualize the articular surface, with lower incidence of wound complications. Different implants have been used to fix calcaneal fractures, including wires, screws and plates with different shapes. Patients and methods: in this study we had 30 patients with 33 calcaneal fractures. Only 30 fractures met the inclusion criteria. Mean age was 37.67 years. In this study only patients with joint depression type were included. 40% of the cases were sanders type 2 and 60% of the cases were sanders type 3. All the patients were treated through Sinus tarsi approach. The articular surface was reduced under vision in all cases.15 cases were fixed by 4.5 mm screws, and 15 cases were fixed by 3.5 mm reconstruction plate. Results: The mean AOFAS ankle / hindfoot score was 81 in the plate group and 82.4 in the screws group.The mean follow up duration was 4.5 month.Complications included two cases with wound infection and two cases of hardware irritation in the plate group. Also complications included four cases of hardware irritation and two cases of fibular impingement in the screws group.
