Abstract

In this study we evaluated mineralized plasmatic matrix with and without collagen membrane in peri-implant bone regeneration in anterior maxillary horizontal alveolar defects with delayed implant placement in the aspects of postoperative pain levels which was measured by visual analogue scale, the postoperative edema which was assessed by the ability of pitting , post-operative peri-implant probing depth which was evaluated by Gallagher and Silver Probing pocket depth and post-operative implant stability which was measured by Osstell and radiographic evaluation using CBCT of post-operative peri-implant bone loss, peri-implant bone density and buccal bone thickness.
In this study,16 implants in 15 patients (10 males and 5 females) with missing anterior maxillary teeth and having deficient alveolar thickness were included. Patients were selected from the outpatient clinic of the Oral and Maxillofacial Surgery Department, Faculty of Dentistry, Alexandria University and divided into two groups; In (group I) 8 implants were placed and the defect was completely covered by MPM, and in (group II) 8 implants were placed and the defect was completely covered by MPM with collagen membrane.
Before all surgeries the patients were clinically examined and preoperative panoramic x-rays and CBCT were taken.
Before surgeries the oral cavity was prepared by 0.12% chlorhexidine mouth rinse solution for thirty seconds. With the patients under local anaesthesia a full thickness mucoperiosteal para-crestal flap was raised to expose the surgical site. Implant was placed

according to the surgical protocol suggested by the manufacturer. After implant placement, the buccal bone defect was grafted using MPM with or without collagen membrane. Flap was sutured using 3/ 0 silk suture. In group I: after implant placement, the buccal bone defect was grafted with MPM while, in group II: after implant placement, the buccal bone defect was grafted with MPM and covered by collagen membrane.
Patients were evaluated clinically for presence of pain and edema in first week, presence of infection every month for the following 3 months, peri-implant probing depth was evaluated after 3,6,9 months and implant stability evaluated immediately post-operative and after 3 months. all patients were evaluated radiographically using CBCT after 3 and 9 montha respectively for recording peri-implant bone loss, peri-implant bone density and buccal bone thickness.
After preforming the statistical analysis, it was found that there was no statistically significant difference in bone regeneration between the two groups but there was a statistically significant increase in implant stability, bone density and buccal bone thickness between follow up periods in each group.
