Abstract

Introduction: Febrile neutropenia is an important cause of mortality and morbidity in pediatric oncology patients receiving chemotherapy. Aim: Assessment of the different factors related to febrile neutropenic episodes in pediatric patients on outcome; as regards clinical, microbiology patterns and response to empiric antibiotics with applying escalation and de-escalation process according to guidelines at the Pediatric Oncology Department, National Cancer Institute, Cairo University. Materials & methods: Three hundred and ninety-four children from the first of June to end of December 2016 were followed prospectively. The demographic characteristics, laboratory data, infection foci, and frequency of microorganisms grown in culture were examined. Also, initial empiric treatment & modifications which were defined as addition or shifting to other antimicrobials and were made according to clinical condition and/or results of blood culture and sensitivity. Results: Of the total of 702 episodes, 536 (76.4%) episodes occurred in hematological malignancies while 166 (23.6%) were in solid malignancies,116 (16.5%) were fever of unknown origin, 350 (49.8%) were clinically documented infection, and microbiologically documented infection were in 236 (33.6%) episodes.The most frequently clinically documented focus were mucositis (24.1%) and typhlitis (18.7%)
