Abstract

Background: Achilles tendon is one of the largest tendon in the body. However, closed rupture of Achilles tendon is not rare in sports activity in adults. There are several methods to treat Achilles tendon. Each one is associated with complications. The worst complication and the one that is considered a failure of the treatment is Achilles rerupture. Our study aimed to pooling the randomized control trial comparing operative management versus conservative as regard the complications and the outcomes, to address the best management available. Materials and methods: we searched many databases (PubMed, scholar, ISI, Embase, Scopus). The search terms included (operative, percutaneous, surgical, conservative, non-operative).The search results were screened for title abstract and full text for eligibility to the inclusion criteria which was published in the online protocol registered at Prospero. The screening and data extraction was independently performed by reviewers. Results: Twelve studies were included in the analysis with 1059 patients.Open operative management was associated with lower risk of rerupture in comparison with conservative measurement (risk ratio 0.35 ,95% confidence interval [0.20,0.59], Pvalue=0.0001), however the risk of rerupture was statistically insignificant between the two groups if accelerated functional rehabilitation was adopted
