Abstract

Objective: To compare the effect of a combination of letrozole with misoprostol versus misoprostol alone in successful induction of medical abortion. Methods: A randomized controlled trial was planned to involve 150 women. They were randomly divided into 2 groups; each group contained 75 participants. Case group (letrozole group) received pretreatment letrozole 12.5 mg for 2 days while control group (misoprostol only group) received only misoprostol. Both groups received misoprostol in a dosage according to the American College of Obstetrics and Gynecology (ACOG) guidelines based on gestational age; 800 og single dose sublingual in the first trimester; and 400 og sublingual every 3 hours for up to 5 doses in the second trimester Results: In total 137 women completed the study; 69 in the letrozole group; and 68 in the misoprostol only group. Complete abortion was documented in 96 cases (70.07%); 51 in the letrozole group (73.9%) and 45 (66.2%) in the misoprostol only group (P=0.32). However, the mean interval to initiation of expulsion in the case and control groups was 7.5±1.8 hours and 8.3±1.5 hours respectively. This indicates a significantly shorter duration to initiation of expulsion in the case group (P=0.003)
