Abstract

Objective: The aim of this study was to compare the diagnostic accuracy of 3D-SIS with of{uFB01}ce Hysteroscopy (control) regarding uterine cavity assessment after hysteroscopic septum resection and their agreement in uterine cavity assessment after hysteroscopic septum resection Methods: 50 patients with uterine septum confirmed by 3D-SIS, complaining of infertility or recurrent miscarriages were recruited from infertility clinic, Kasr Al Ainy hospital, Faculty of medicine, Cairo University over 1 year from February 2019 to February 2020. Hysteroscopic septum resection was done at Kasr Al Ainy hospital operative theatre. Eight weeks later, 3D-SIS was done during the postmenstrual phase by the same investigator for all patients, it was done on the same day immediately before second look office hysteroscopy done by the same investigator using calibrated instruments and patients did not receive any medication between the procedures. Outcome measures: Preoperatively, length, width of septum and residual cavity lengths were assessed by 3D-SIS. Postoperatively, length of fundal notch (residual septum), residual cavity length, and presence or absence of intrauterine adhesions were assessed by 3D-SIS and office hysteroscopy.Primary outcome: comparison between the results of the 3D-SIS and office hysteroscopy done after hysteroscopic septum resection and agreement of 3D-SIS in evaluation of uterine cavity postoperatively. Secondary outcome: comparison between the results of the 3D-SIS done before and after the septum resection. Results: our study showed no significant differences between results of 3D SIS and office hysteroscopy regarding length of fundal notch (P value 0.215) and residual cavity length (P value 0.661), and it showed very good agreement between their results (P value <0.001). Regarding intrauterine adhesions detection, 3D-SIS showed sensitivity 55%, specificity 100%, PPV 100%, NPV 91% with 92% accuracy and good agreement to the results of the office hysteroscopy by Kappa method (K=0.672)
