Abstract

The present study is an observational cross sectional study aimed to evaluate the levels of serum (CXCL10) in RA patients with ILD and those without ILD who attended inpatient and outpatient Clinic at the Rheumatology, Rehabilitation and Physical Medicine Department, Assuit University Hospital, Assuit, Egypt in a period from the start of January 2019 up to the end of December 2020. The study included 100 RA patients (50 with ILD and 50 without ILD).
Patients with RA-ILD being older with longer disease duration compared to RA patients without ILD. Meanwhile other demographic variables and environmental factors were comparable between both groups with no significant difference between them. Also RA-ILD patients suffered from higher cough, pain, expectoration and dyspnea, and more restricted PFT, with significantly higher disease activity, and lower quality of life, also patients with RA-ILD have significantly higher ESR, CRP, and CXCL10.
Among patients with RA-ILD; the mean of CXCL10 was significantly higher with high disease activity group. Also we found significant positive correlation between the serum level of CXCL10 and the age, disease duration (years), DAS 28 score, ESR, CRP and RF of the studied patients, and significant negative correlation with MHAQ.
At a cut off 128.15 pg/mL, CXCL0 biomarker has a sensitivity of 88.0% and a specificity of 70.0%, areas under the ROC curves was 80.7% (95%CI: 0.72 - 0.894, P=0.000) for prediction of RA-ILD.
