Abstract

This is a prospective comparative interventional study carried out on forty patients presenting at the ENT out-patient clinic seeking rhinoplasty for dorsal nasal hump deformity, during the period from February 2020 till August 2021.They were randomly divided into two equal groups of twenty patients as follows; group (A) patients underwent dorsal hump remodeling and reinsertion while group (B) patients had hump resection with no dorsal grafting. All surgeries were done through the open or closed approach by the same surgeon. The results were compared functionally and esthetically. Functional assessment was done subjectively by the NOSE score and objectively by the active anterior rhinomanometry. Esthetic results were based on patient{u2019}s satisfaction of the esthetic outcome using VAS scale and objective assessment of the nasal width in frontal view photography post-operative compared to the pre-operative photos. It was concluded that hump remodeling and re-insertion technique is more superior in the esthetic outcome with more natural looking dorsum and narrowing of the dorsal nasal width. Additional advantage of hump re-insertion is the decrease in the need for osteotomies and avoiding their consequences. Hump reinsertion is useful as a rescue procedure in case of excessive dorsal resection. Both hump re-insertion and hump resection techniques have nearly equal impact regarding the functional outcome of surgery
