Abstract

Malignant pleural effusion (MPE) imposes a significant burden onpatients and health-care providers, So a prompt diagnosis is crucial to allow patients to start optimal treatment as early as possible. Management of malignant effusion depends on palliation of dyspnea and prevention of the re -accumulation of pleural fluid to provide the highest possible quality of life This prospective interventional clinical trial study, was conducted in the period from July 2019 till July 2021 it was carried out on 100 patients with MPE were enrolled and randomly subdivided into four equal groups based on type of material used for pleurodesis; group 1: spontaneous pleurodesis by the intercostal chest tube, group 2: pleurodesis with tetracycline solution, group 3: pleurodesis with tetracycline poudrage and finally group 4: Pleurodesis with iodopovidone The study aimed to compare efficacy and success rate of different materials. Also, to compare chest sonographic findings versus multislice CT findings in patients with malignant pleural effusion, and compare sensitivity of chest sonography versus multislice CT in detection presence of thick pleura and pleural thickening Also, aimed to compare between the four groups mentioned above used in pleurodesis to detect the success of each modality.Mean age of enrolled patients was 56.87 ± 22.87 years and majority (59%) of them was 25-64 years old. Also, it was found that 11(11%), 59(59%) and 30 (30%) patients were, 18-24 years, 25-64 years, and ≥ 65 years old, respectively. Out of the studied patients; 57 (57%) patients were males and the other were females. Sixty (60%) patients were smokers, 31 (31%) patients were ex-smokers while 9 (9%) non smoker. Based on baseline radiological data (CT finding), it was found that pleural nodules, thickening and intrapulmonary mass are present in 85 (85%), 71 (71%) and 34 (34%) patients, respectively. While by chest sonography we found that pleural nodules, thickening and intrapulmonary mass are present in 50 (50%), 75 (75%) and 28 (28%)patients, respectively . Mean amount of aspirated fluid was 3305 ± 634.23 ml It was found that 81 (81%) patients didn’t report any complications following the procedure while five patients suffered from fever and another 13 (13%) patients were complaining of pain. One patients developed anaphylaxis and controlled by medical therapy. Generally, 46 (52.3%) patients had successful pleurodesis.
