Abstract some parameters obtained from or calculated by the routine blood tests including fibrinogen and platelet indices; mean platelet volume, platelet distribution width and platelet large cell ratio can serve as independent risk factors in metabolic diseases as diabetes mellitus and indicators of microvascular complications. The purpose of the study: was to investigate whether these parameters; fibrinogen and platelet indices; (mean platelet volume, platelet distribution width and platelet large cell ratio) are useful variables for determining micro vascular complications in type 2 diabetes. Methods: The study population consisted of 90 subjects divided into 3 groups. group included 30 uncomplicated diabetic subjects. group included 30 diabetic patients with microvascular complications. group included 30 healthy subjects, all were submitted to full history taking, full clinical examination and routine laboratory investigations; (CBC, fasting blood glucose, 2-hour post prandial blood glucose, glycated hemoglobin, liver and kidney functions). Specific investigations (fibrinogen, mean platelet volume, platelet distribution width and platelet large cell ratio). In addition, specific tests for micro vascular complications as fundus examination and so, neuropathy was evaluated by; testing sensory perceptions of light touch, pain sensation, vibration sense, temperature sense and ankle jerk reflex.in addition, nephropathy was evaluated by urine analysis for detection of albuminuria. Results: fibrinogen was significantly higher in the complicated group compared to the other studied groups (p < 0.001).Mean platelet volume was significantly higher in the complicated group compared to the other studied groups (p < 0.001). Platelet distribution width was significantly higher in the complicated group compared to the other groups also platelet large cell ratio was significantly elevated in the complicated group compared to the other studied groups (P<0.001). Conclusion: Serum fibrinogen, platelet distribution width and platelet large cell ratio can be used as simple parameters to detect micro vascular complications in type 2 diabetes mellitus than mean platelet volume. |
