Abstract

Vitiligo is a frequent acquired disease that results from the loss of melanocytes in the epidermis of the skin. Clinically, it appears as depigmented macules or patches of skin. The global incidence is estimated to be between 0.5 and 1 percent. Vitiligo, in addition to the physical deformity, may have a significant impact on a patient’s mental health, particularly if it occurs on exposed or aesthetically essential portions of the body such as the face or hands. Different therapy approaches have been created, although with varying outcomes, most of them are undesirable for the patients and are of extensive duration and duration.
These findings open the door for combination treatment, which has shown to improve repigmentation response when compared to monotherapy.
Latanoprost, a synthetic prostaglandin analogue that lowers intraocular pressure and has been linked to periocular skin hyperpigmentation, probably owing to enhanced melanogenesis, is one of the therapy options.
Fractional CO2 therapy is another therapeutic option that has showed promising outcomes in many clinical studies. The fractional CO2 laser is thought to accelerate melanin cell migration and differentiation of stem cells.
Another treatment option is PRP, which is an autologous preparation of platelets in concentrated plasma with a high concentration of growth factors. Cell migration, adhesion, proliferation, and differentiation are all regulated by these growth factors.
HMB-45, a monoclonal antibody specific for melanocytes, particularly premature developing melanocytes, was used to detect melanocytes.Our study was the first study compare the combination of the three modalities of treatment topical Latanoprost 0.005%, PRP and/or fractional CO2 laser.
The aim of the this study was to evaluate the effectiveness of topical Latanoprost 0.005% (Ioprost®) in treatment of vitiligo, and compare its effectiveness combined with PRP and/or fractional CO2 laser in treatment of localized stable vitiligo.
The present study was carried out on 60 patients with localized stable vitiligo. This study was a prospective randomized study, parallel group, comparative study, including sixty adult patients with stable vitiligo was enrolled in the study. They were recruited from the Outpatient Clinic of Dermatology and Venereology and Andrology Department, Main Alexandria University Hospital, Faculty of medicine, Alexandria University, Alexandria, Egypt.
The patients were divided into three groups each of 20 patients:
group I: Includes patients will apply topical latanoprost drops only.
group II: Includes patients will apply topical latanoprost drops and 4 fractional CO2 laser sessions at 2 weeks interval.
group III: Includes patients receiving latanoprost plus fractional CO2 plus platelet rich plasma.
