Abstract

Background: Diabetic kidney disease (DKD) is a chronic vascular complication of DM with high prevalence despite of rigorous treatments. Better diagnostic methods, prognostic markers and treatment choices are still instantly required to assist in better management of DKD. CB1R is the most prominent receptor in the kidney. Emerging evidence also advocates a key role of CB1R signaling in inducing inflammation and fibrosis in DKD. MicroRNAs are single-stranded, short (19{u2013}25 bases), non-coding RNAs which have been drawing attention as important regulators of gene expression. CircRNAs are a group of endogenously expressed regulatory RNAs with a single-stranded circular structure. The most well-known function of circRNA is that it can act as a sponge for miRNAs and proteins controlling gene expression. This study aimed to investigate the emerging role of hsa_circ_0000146 and hsa_circ_0000072 in regulating cannabinoid receptor- 1 gene expression through sponging hsa-miR-21, hsa-miR-495 respectively in DKD in Egyptian patients. Methods: The study was conducted on 3 different groups. 55 Healthy volunteers (GroupI), 55 Diabetic patients (group II) and55 DKD patients (group III). The gene expression level of CNR-1, hsa-miR-21, hsa-miR-495, hsa_circ_0000146 and hsa_circ_0000072 was assessed by RT-PCR. In addition to ELISA technique was used to assess CNR-1and CRP protein level Results: Our study revealed a significant increase CNR-1, hsa-miR-21 and hsa-miR-495 gene expression in DKD patients compared to diabetic patients and control. There was a significant increase in CNR-1 and CRP proteins level in DKD patients compared to diabetic patients and control as well. In addition, we found a significant decrease in hsa_circ_0000146 and hsa_circ_0000072 gene expression in DKD patients compared to diabetic patients and control
