Abstract

Postoperative cognitive dysfunction (POCD) is a common complication of the central nervous system (CNS) after surgery. It is characterized by impaired memory, orientation, perception, attention, verbal articulation, and abstract thinking. The current study was conducted at Mansoura university Hospitals aiming to study the effect of dexmedetomidine administration on neurocognitive dysfunction, hemodynamics, sedation and post-operative recovery after total laryngectomy in elderly population. We included a total of 182 cases who were randomly allocated into two equal groups ; Dex group included 91 cases who received dexmedetomidine infusion (started as 1 µg/kg bolus infused over 10 minutes then it was maintained on 0.2-1.4 µg/kg/hour), and Control group which included the remaining 91 cases who were considered controls. Results revealed that Dexmedetomidine administration is associated with a significant improvement of cognitive function after surgery in the elderly population. It is associated with a better analgesic and sedative profile along with decreased neurological inflammatory markers (S100B). However, the patient must be closely monitored for side effects like bradycardia and hypotension. Our study has some limitations; First, it is a single center study. Also, the postoperative follow-up period was relatively short. Hence, more studies including more cases from different medical centers with long-term follow-up should be conducted in the future.
