Abstract

Coronary artery diseases(CAD) is the leading cause of death in Western world and it is the most costly cardiovascular disease (CVD) in terms of physician services, hospitalizations, and lost productivity due to premature death. Invasive coronary angiography (ICA) is the procedure of choice for the diagnosis of coronary artery disease (CAD). ICA provide a clear visualization of the coronary arterial blood flow, which helps in the diagnosis and treatment decision. However, single positron emission computed tomography (SPECT) emerged as the first method that evaluated myocardial perfusion directly, that was non-invasive, readily available, and with an adequate cost-benefit relationship. Because of that, SPECT promptly positioned as one of the gatekeeper tests for patients with symptoms of myocardial ischemia. Objective: The aim of this study is to assess the accuracy of exercise MIBI single photon emission computed tomography (SPECT) in the evaluation of the extent of coronary artery disease in comparison with coronary angiography as the gold standard. Patients and methods: This was a prospective comparative, single center study carried out between December 2021 and December 2022 and conducted in cardiovascular department at Mansoura Specialized Hospital. It was Carried out on 44 adult patients of both gender with clinical likelihood of coronary artery disease. All patients underwent exercise MIBI SPECT and invasive coronary angiography(ICA). Results: The overall sensitivity of SPECT MPI in detection of obstructive CAD using cut off value of diameter stenosis ≥ 50% in any of major epicardial coronary arteries is 90.3% , 61.5% specificity and total accuracy is 81.8% . Conclusion: Exercise myocardial perfusion SPECT is a well validated, non-invasive method for assessment of myocardial perfusion in men and women. It has the ability to localize hemodynamically important coronary stenosis, and assess the extent and severity of coronary obstruction
