Abstract Since the beginning of the recorded time, the breast has been a symbol of motherhood, femininity, and sexuality. It has been portrayed throughout history in works of art symbolizing each of these aspects of a woman’s life. the breast has been memorialized as a central focus of a woman’s anatomy. Breast cancer occurs in both genders and consider as the most frequent type of cancer in female all over the world. The incidence increases dramatically with age. It is clear that the psychological impact on women undergoing mastectomy is profound, and includes body image changes as well as many other emotional challenges that must be addressed in order for successful adaptation to a “new way” of life. Needed to removal of breast tissue is limited in conditions as breast cancer, huge fibroadenoma, calcified fat necrosis post trauma. Breast conservation therapy (BCT) is the preferable treatment for women with early stage breast cancer. Overall survival of women undergoing BCT is equivalent to mastectomy. Oncoplastic breast surgery initially originated in order to improve the esthetic result of breast conserving surgery (BCS), the indications of which have grown significantly in recent years. The optimal choice regarding the best oncoplastic solution depends on the proportion between the resected tissue volume and the whole breast volume as well as on the breast lesion site. Breast reconstruction offers restoration of breast consistency to such women, and make the breast similar in size, shape, contour and position to the opposite breast. Thus, women restore the option to wear any clothing without needing external breast prostheses and have decreased psychological effect than those who have mastectomy without reconstruction. The women have several options for reconstruction. One of this options are implants as silicone, another options by using autologous tissue like as ,local flaps, pedicled flap and free vascularized flap and autologous tissue transfer such as, lipofilling & free dermal fat graft, also both implant and autologous tissue can using together. There are two types of fat graft: lipofilling and dermal fat graft. Lipofilling: procedure consist of: lipofilling then converted to liquid form after decanting and centrifugation to separate debris, excess fluid, and dead cells from the viable adipose fat cells. Then Re-injected into the breast. |
