Abstract

Obesity is defined as the excessive or abnormal accumulation of fat within the body which will impair health. Obesity is a common problem has become a plague which has worsened for the last few years and causes an economic burden in some countries
Obesity is a complex disease of multifactorial etiology.
Obesity is combined and associated with multiple medical conditions and can cause serious complications of chronic conditions. Obesity needs multi prolong treatment strategies and may require lifelong treatment. A slight weight reduction (5% to 10% weight loss) can significantly improve health, quality of life, and economic burden of an individual and a country as a whole.
Sleeve gastrectomy is a famous, common and popular surgical technique used in bariatric surgery and considered as leading successful method in metabolic surgery. Sleeve gastrectomy has proved to be a successful method in reducing weight.
There are some disparate effects that patients may experience after sleeve gastrectomy including a reduction in BMI, weight and treatment of associated co morbidities e.g. treatment of high blood pressure, stroke. Sleeve gastrectomy has a significant effect on remission of obesity-related diseases including type 2 diabetes mellitus (T2DM), Non-alcoholic fatty liver (NAFLD), cardiovascular disease, obstructive sleep apnea.
The complications of the procedure of sleeve gastrectomy include bleeding either intraoperative bleeding or post-operative bleeding, nutrient deficiencies, stenosis, gastroesophageal reflux, vomiting and leakage.
Sleeve gastrectomy achieve weight reduction and improvement in glucose metabolism by many mechanisms including: restricted caloric intake, changes in hormonal secretion of gastrointestinal tract, changes in energy expenditure, rearrangement of hypothalamic control and changes in rhythm and control of vagal tone.
Our study included 30 diabetic patients 4 men (14%) and 26 women (84%).our study showed improvement of diabetes mellitus in 24 patients (80%) and 6 patients (20%) no improvement .Among patients improved 21patients (70%) show complete remission of diabetes (patients stopped medications), 3 patients(10%) controlled by oral hypo glycemic drugs (patients decrease medications).
Our study showed that there is a definite improvement in the clinical status of diabetes as seen by resolution of diabetes in some, and reduction in doses and number of antidiabetic drugs.
Patients not improved (6 patients (20%)) may be due to patients are long duration of diabetes, family history, patients on multiple medications and did not follow instructions of doctors (no compliance) at follow up visits, bad eating habits, insufficient follow up time.
