Abstract

Pseudo-exfoliation syndrome is a common age-related disorder characterized by production and progressive deposition of a fibrillar extracellular material in ocular and extra-ocular tissues. The cornea may be more susceptible in presence of endotheliopathy to the effects of phacoemulsification surgery.
Increased risk of complications of cataract surgery and presence of endotheliopathy may lead to endothelial decompensation with higher endothelial cell density loss and more corneal swelling in eyes with Pseudo-exfoliation.
In this case-controlled study, 66 patients of both sexes were included & done in the Department of Ophthalmology, Ain Shams University Hospitals and had informed written consent in accordance with Ain Shams Ethical Committee Regulation and divided into two groups;
1- Study group: 33 patients with Pseudo-exfoliation.
2- Control group: 33 patients with senile cataract without Pseudo-exfoliation.
The two groups are subjected to detailed medical history, slit lamp examination and examined by Specular microscopy to evaluate endothelial cell count pre-operative and 4 weeks post-operative.
There was no significant difference between two groups regarding age, sex, medical history, best corrected visual acuity, slit lamp examination, fundus examination and biometry.
Regarding endothelial cell count, there was a highly significant difference between two groups pre-operative and 4 weeks post-operative, while there was no significant difference in percentage of endothelial cell loss.
There was high significance in correlation between endothelial cell count pre-operatively & post-operatively (4 weeks) regarding age, best corrected visual acuity and biometry, while there was no significant difference between endothelial cell count pre-operatively & post-operatively regarding sex, medical history, slit lamp and fundus examination.
In Pseudo-exfoliation syndrome, due to a lower functional reserve of endothelial cell density pre-operatively together with factors such as shallow anterior chamber, small pupil and zonular weakness that increase the level of difficulty of cataract surgery, anterior chamber manipulations intra- operatively should be performed with care at a safe distance away from the corneal endothelium to prevent complications and Endothelial cell denisty loss post-operatively.

Recommendations
In pseudo-exfoliation cataract surgery, anterior chamber manipulations intra- operatively should be performed with care at a safe distance away from the corneal endothelium to prevent complications and endothelial cell density loss post-operatively.
