Abstract

Background: Obesity is on the rise all over the world. It is the disease of our modern age that is capable now of producing more morbidity and mortality than ever. Surgical treatment of morbid obesity is now considered the standard of care compared to less effective pharmacological treatment.
Aim of the Work: To evaluate the outcome of total measurement of bowel and leaving two thirds of it as the common channel, compared with proximal fixed measurement in One Anastomosis Gastric bypass surgery regarding effects on weight loss and nutritional status, to determine if total measurement of bowel should be adopted as a standard technique.
Patients and Method: In our study we compared two techniques of surgery by randomizing for two equal groups; the first group had the conventional method with bypass of 200 cm of small bowel and the other group underwent total small bowel measurement and bypass of the proximal one third leaving two thirds of the bowel as the common channel. We compared the two groups regarding weight loss, serum iron, total calcium level, serum albumin level, vitamin B1 level, vitamin B12 level and vitamin D level.
Results: The results we obtained of the study confirmed that measurement of the total bowel length and bypass of a fixed ratio of it (in our case, one third) produced better weight loss results with better overall nutritional status and less nutritional deficiencies. Thus, we recommend this as a method of performing OAGB till more studies with larger sample sizes become available.
Conclusion: Obesity is a true problem of our age that is the focus of the scientific community nowadays. One-anastomosis gastric bypass has gained a lot of interest in the past years and is now considered the third most practiced bariatric surgery in the world.
