Abstract

Purpose:
To evaluate foveal avascular zone (FAZ) and macular perfusion in eyes with diabetic macular edema (DME) pre and post treating with single intravitreal anti-VEGF injection by optical coherence tomography angiograph.
Methods:
Treatment naïve patients with DME were prospectively recruited from Cairo University retina clinic. Included patients underwent full ophthalmological examination and investigation by SD-OCTA at baseline and a month after intravitreal anti -VEGF injection. 6X6 mm macular scan protocol was used. Pre and post-treatment OCTA images were automatically aligned using a commercially available retina alignment software (i2k Align Retina software) then the FAZ , vascular density (VD) changes following the injection were obtained at the full retinal thickness.
Results:
A total of 40 eyes of 26 patients successfully completed the 6X6-mm scan protocol. Using the 6X6-mm protocol, there was 8.1% increase in FAZ area, 1.3% decrease in FD-Full and FD-SCP, 1.9% decrease in FD-DCP, 8% decrease in VD-Full, 9.1% decrease in VD-SCP, 10.6% decrease in VD-DCP, 13.3% decrease in skeleton VD-Full, 12.5% decrease in skeleton VD-SCP, and 16.3% decrease in skeleton VD-DCP following injection which were all statistically significant (p<0.05).
Conclusion:
OCTA could be a useful non-invasive tool for the evaluation of changes of the macular perfusion following treatment of DME. Randomized controlled trials using OCTA to compare the effect of various treatment modalities for DME on macular perfusion are needed.
