Abstract

The AFC is one of the more commonly measured parameters linked to the ovarian function. However, until now the interpretation of the measurement was mainly based on the individual experience of the operator, because no normal data were present Hence, the establishment of a nomogram of AFC values is the first step to counsel patients on a scientific basis, also there is Limited data is available on ovarian aging in the healthy population and the role of sonographic biomarkers (AFC) of ovarian reserve. Most of the available data is based on studies outside the Egyptian context. The present study, therefore aim was to examine the relationship between antral follicle count (AFC) with age throughout the reproductive period and construct age-related nomograms among a sample of general population in minia locality and to assess whether available AFC nomograms present any geographical inconsistency.
To achieve this study a cross-sectional study was conducted in the Department of Obstetrics and Gynecology, faculty of Medicine, Minia University in the duration from January 2020 till December 2020. In the outpatient clinic, 315 premenopausal women with regular menstrual bleeding were recruited for the study, Data from 123 patients were available for the statistical analysis All patients with eligible criteria where recruited in the study AFC was measured by transvaginal ultrasound examination.
Estimation of the relationship between AFC and age and developing the AFC nomogram was done, the analysis showed a linear decline in AFC with age; for every year increase in age, the median AFC decreases by 0.83 per year. The AFC corresponding to the 5th, 25th, 50th, 75th, and 95th centiles for each age have been calculated.
The median age (25th–75th centile) of women was 27 years (23–35) and median (25th–75th centile) of the AFC was 18 (12–22).
AFC was correlated with age; Pearson correlation was done to test for significance between the AFC and age and it showed a significant correlation between (P<0.001). It was a Negative Strong correlation (R= -0.90).
Regression analysis revealed that age-related changes were best fitted by a linear function (B= -0.83; P < 0.001).
Age alone accounted for 81.8% of the variation in AFC.
Age explained the decline in AFC by 0.83 per year in general population, Age explained the decline in AFC by 10.6 per year after the age of 30 years and the decline in AFC by 11.49 per year after the age of 35 years.
In conclusion the nomogram of Antral Follicular Count (AFC) in relation to age throughout reproductive period in a sample of general population of women in minia locality including the 5th, 25th, 50th, 75th, and 95th centiles for each age have been calculated and plotted in one figure which may be the first step to counsel patients on a scientific basis.
