Abstract

Background: Although normal ET function is essential for maintaining the normal aeration of tympanic cavity and the health of the middle ear mucosa, there is lack of consensus about the effect of the ET dysfunction on the surgical outcome of tympanoplasty. Some authors concluded that tympanoplasty relieves the inflamed mucosa of the middle ear and improve the middle ear mucosa, they though that ET dysfunction has no effect on the outcome of tympanoplasty. This lack of consensus may be due to absence of a single gold standard test that can measure the ET function accurately which may be referred to the complexity of the structure the ET. Thus, combination of the ET function tests may be helpful in better assessment of the ET function. Aim of this study: The aim of this study is to find the relation between the ET function and the surgical outcome of type 1 tympanoplasty in chronic suppurative otitis media (CSOM) inactive mucosal type. Method: This study is a prospective clinical case series study conducted on 64 patients with CSOM inactive mucosal type in the Otorhinolaryngology Department, Mansoura University Hospital in the period between November 2019 till October 2020 with inclusion and exclusion criteria Results: The saccharine test showed a good correlation with the graft healing and the hearing outcome. About 91.67 % of patients with normal ET function and 69.23% of patients with partial dysfunction showed intact TM without retraction while all three patients who had gross ET showed graft failure. The methylene blue test showed no correlation with the surgical outcome of the tympanoplasty. Intact TM without retraction was obtained in 85.45% of patients with normal ET function and 66.67% of patients with partial dysfunction. Residual perforation occurred in three patients with normal function and one patient with partial dysfunction. The PET showed a good correlation with the graft healing and the hearing outcome. About 60% of patients with gross dysfunction developed graft retraction with postoperative minimal hearing improvement. Conclusion: There is a correlation between the ET function and the results of tympanoplasty type-1. Saccharine test and PET showed a good positive correlation with the graft healing and the hearing outcomes.
