Abstract

This a prospective randomized controlled trial phase III study was
conducted to assess the efficacy and safety of the use of low dose gemcitabine as radiosensitizer concomitant with palliative radiotherapy on the brain in patents with brain metastasis secondary to solid primary tumor the study was conducted in Clinical Oncology and Nuclear medicine department in Suez Canal University Hospital.
40 eligible patients fulfilling the inclusion criteria, had been enrolled in the study in period between December 2018 to December 2020, then they were randomly distributed in two group 1 (CCRH):
includes patients who received Gemcitabine 2–4 h prior to WBRT (30 Gry / 10 fractions) with dose of 50 mg/m2 twice-weekly with a standard 30-minute infusion.
group 2 (RTH only):
includes patients who received conventional treatment in form of WBRT (30 Gry / 10 fractions / 2 weeks).
Then patients were assessed clinically and radiologically one month after the end of intervention then follow up every 3 months or earlier if there is symptoms or signs of progression. The duration of follow up was 12 months.
The study illustrated slight improvement in clinical and radiological response in CCRTH group versus RTH only, in form of better clinical improvement (65 %) and less progression (15%) in CCRTH group than RTH group (55% and 35% respectively). If we considered stable disease in radiological imaging response, so CCRTH group show 85% radiological response versus 65% in RTH group.
